SCHOOL HEALTH FORMS
Below are various health forms that are utilized by the Edmonds School District. These forms need to be filled out and signed by parents/guardians and/or a licensed health care provider and returned to the appropriate school nurse or office staff.

| Camp Health Form | This form is completed by parents for students attending outdoor education camp. If medication needs to be given at camp, a licensed health care provider must sign the form. |
| Exchange of medical information authorization | Gives permission for designated school staff to communicate with the student's Health Care Providers. |
| Immunization Form | |
| Certificate of Immunization Status | Form necessary for parent to document their student's immunizations. |

| Medication Administration Authorization Form | This form is required if a student is to receive medication at school. |
| Self-Carry Inhaler Medication Form | Form that is signed by a licensed health care provider and parent to allow student to self-carry an inhaler at school. |
| Student Self Administration of Asthma Inhalers Letter | This letter explains the new State Asthma Bill that regulates how students can use inhalers at school. |
| Asthma Action Plan | Students' individual asthma action plan to be completed by parent |
