Clock Hour Proposal Form (for a course that has not yet been offered during the current academic year)

Clock Hour Repeat Proposal Form (for a course that has already been offered during the current academic year)

VITA Form (this form should be submitted once every seven (7) years

 

Taking a Class

Pre-Approval Request Form
This form is necessary for courses which you have taken from agencies other than Edmonds School District and should be submitted to Jim Roberts in Human Resources.

Verification Forms (P-120) and Evaluations (P-121):
If your Office Manager does not have any, you can order them through the Print Shop (ext. 7096).

Important Note: These forms are only to be used for courses held by Edmonds School District. If you are not sure whether your course is an Edmonds School District course, ask your instructor.
