Healthy Youth Survey

 

The Healthy Youth Survey is for Grades 6, 8, 10, & 12. It is conducted across Washington State to survey students' opinions on health, alcohol & drug use, nutrition, relationships, etc.  The data is used by schools, communities and the state and local health departments to plan programs to support youth and reduce their risk factors.

Dear Parents,

During the week of October 13 - 17, 2008, students in the 6th, 8th, 10th and 12th grades in the Edmonds School District will be taking part in the Healthy Youth Survey.   This survey is jointly sponsored by the Office of Superintendent of Public Instruction (OSPI); the Department of Health (DOH); the Department of Social and Health Services' Division of Alcohol and Substance Abuse (DASA); the Department of Community, Trade and Economic Development (CTED); and the Family Policy Council (FPC). The survey will be administered by RMC Research in Portland, Oregon, a professional research company.

The Healthy Youth Survey will assess a variety of health behaviors and other important factors in the well-being of young people.   Each student will be asked about his/her own behavior, beliefs   and attitudes about several topics including: alcohol, tobacco, drug use, violence, weapons, neighborhood, community, peer behaviors, illegal activities and school performance.

The survey is entirely anonymous to protect your child's privacy .   Students will not put their names on the survey question or answer sheets.   Results of the survey will be used to plan violence and substance abuse prevention programs in our communities.

Student participation in this survey is completely voluntary.   If you do not want your child to participate in the survey, please fill out the form below and return it to his/her school.   Even if your child participates in the survey, there is no obligation to answer any question he/she does do not want to answer.   Again, this is purely voluntary.

If you would like to review the survey, a copy is available in your school office.   Please contact your school principal with any questions.
